On May 19th, the Office of the National Healthcare Security Administration and the Office of the Ministry of Finance issued the "Notice on Further Strengthening the Supervision and Management of the Use of Personal Accounts for Basic Medical Insurance of Employees in Designated Retail Pharmacies" (hereinafter referred to as the "Notice"), emphasizing that the personal accounts for basic medical insurance of employees adhere to the functional positioning of "protecting the foundation". The Notice requires provincial medical insurance departments to issue a unified whitelist of personal medical insurance account payments for employees of designated retail pharmacies by the end of September 2026. Employees on the whitelist can pay for their medical insurance through their personal accounts at designated retail pharmacies. Non medical supplies such as health care products, daily necessities, toothpaste, toothbrush, dental floss, facial mask, cosmetics, contact lenses, as well as equipment consumables with life functions and low medical added value shall not be included in the white list. In November last year, several media exposed that some designated retail pharmacies sold toothbrushes, toothpaste, dental floss, facial mask, sun masks and other "machine size" products to medical insurance participants. Although these products appear to meet the requirements of medical insurance policies in terms of category, they are actually promoted and marketed as ordinary daily necessities, completely exceeding the expected scope of use of medical devices. There is a gray operation of "surface compliance, substance violation", which seriously deviates from the functional positioning of medical insurance to protect medical necessities. The "Notice" issued by the two departments aims to systematically correct similar issues at the policy-making level, clarify payment policy boundaries through whitelists, and resolutely prevent personal accounts from being used to pay for products that do not meet the basic medical insurance "basic protection" function positioning while fully meeting the reasonable medication needs of the masses. The medical insurance fund is the people's medical expenses, and the safety of the fund is related to people's livelihood and social stability. The employee basic medical insurance fund consists of a pooling fund and individual accounts. The individual account is an important component of the employee basic medical insurance fund, which can be used not only for the insured person's own medical expenses, but also for the medical expenses of close relatives who have already been insured; Not only can family solidarity be achieved within the province, but cross provincial solidarity can also be achieved; Not only can it be used to pay medical expenses, but it can also be used for personal payment of basic medical insurance for urban and rural residents by close relatives; Not only can it be used to pay for medical expenses incurred at designated medical institutions, but it also allows payment for influenza vaccine expenses incurred at designated medical institutions and compliance fees incurred at designated retail pharmacies. Reasonably, prudently, flexibly and appropriately regulate the use of personal accounts for employee medical insurance, and prevent unreasonable expenses that do not fall within the scope of basic medical security from being included in the expenditure scope, which is crucial for maintaining the security of the medical insurance fund. Pharmacies are the hardest hit areas where personal accounts of medical insurance funds have been 'ripped off'. Several years ago, some pharmacies had already deviated from their duties, with shelves displaying not only toothpaste and cosmetics, but also daily necessities such as rice cookers, rice flour oil, and milk powder. Pharmacies had become nothing but "grocery stores". After multiple rounds of special governance, such chaos has been somewhat restrained. However, according to media exposure, there are still illegal elements coveting medical insurance funds, and the illegal means are becoming increasingly covert, showing a trend of resurgence. Unlike the primitive model of directly putting daily necessities on shelves and blatantly brushing medical insurance cards in the past, the routine of "medical insurance cards turning into shopping cards" has now been upgraded to a new variant with stronger confusion. Some ordinary daily necessities are labeled with medical equipment "vests" and transformed into "medical products" for sale in pharmacies - toothpaste priced at a few yuan can be priced at tens of yuan by changing into "clothing". What is even more alarming is that many insured people hold the mentality of "not wasting money in their personal medical insurance accounts", which invisibly becomes a link in the chain of medical insurance funds, further promoting the spread of such violations. Allowing the medical insurance fund to be used in its rightful place is not only a clear provision of the Social Insurance Law, but also a prerequisite for ensuring the safe, efficient, and reasonable use of employees' personal medical insurance accounts. In this context, the National Healthcare Security Administration has issued the "Five Year Action Plan for Supervision and Inspection of Medical Security Funds (2026-2030)", which aims to continuously consolidate the high-pressure situation of "not daring to cheat", weave a dense network of "not daring to cheat", improve the long-term mechanism of "not wanting to cheat", and comprehensively promote the transformation of medical insurance supervision from "passive response" to "active prevention and control".
